you pay a deposit of 100% of the equipment value, all new accounts must complete a credit
card approval form. Decision for credit approval is based upon your credit report.

form, completely fill it out, and fax it back to our new accounts department at
407-926-1841. Your credit card will not be billed until your account has been approved and
we are ready to deliver your items. If you have questions regarding the form, either
