| RENTAL APPLICATION
Unless you pay a deposit of 100% of the
equipment value, all new accounts must complete a rental application. Decision for credit
approval is based upon your credit report.
Click on the appropriate rental application below. Print the
application, completely fill it out, and fax it back to our new accounts department at
407-926-1841. Your credit card will not be billed until your account has been approved and
we are ready to deliver your items. If you have questions regarding the application,
